How to Customize a Babysitter Information Sheet
A useful babysitter sheet answers the questions that come up after you leave: who to call, who may pick up, what the child normally eats, what bedtime looks like and what should trigger an immediate call. Customize the editable version once, then update it for each visit.

Only include information the sitter needs. Share door codes, payment details and other sensitive access information through a separate private channel.

Put urgent information first
Keep the emergency page short enough to scan. List parents, a nearby backup adult, pediatrician or clinic, poison help and emergency services in the order appropriate for your family.
- Confirm every phone number before the visit.
- Name authorized pickup adults clearly.
- Copy allergy or medication directions from parent and clinician instructions rather than improvising them.
Describe the child’s normal routine
Replace vague wording such as “usual bedtime” with times, steps and comfort items. Include meals, screen rules, bathroom help and anything that prevents a predictable problem.
Make a fresh visit plan
Keep stable family information in the editable master. Before each visit, update the date, expected return, activity plan and food already prepared. Export a new PDF rather than writing over an old date.
Review privacy before printing
Preview every page and remove fields you do not need. Store old printed copies securely because they can contain child, medical and contact information.
